Archive: Nov. 5 Webinar on eMOLST Collaborative Effort

Situation Report | November 9, 2020

Our latest webinar in the Hospital-Home Care Collaboration Series featured NYU Langone and Visiting Nurse Service of New York who shared their joint work to leverage Medical Orders for Life-Sustaining Treatment (MOLST) and eMOLST during the COVID-19 pandemic to ensure patient preferences are honored.

If you missed the live webinar last week, an archive is posted on our Collaboration webpage. There, you can also find archives of three other webinars in the series and downloadable calendar reminders to join the remaining three webinars in the series coming up on November 19, December 3 and December 17.

Our next session, on November 19, will feature Gurwin Health Care System, Stony Brook Hospital and Stony Brook Physician Practice discussing their Integrated Care and Care Management Collaborative. Using virtual connections, telehealth monitoring, and a clinical dashboard, home care providers and physicians monitor pulse-ox, cardiac tests and other readings or reports for care management of patients diagnosed with COVID-19, congestive heart failure, chronic obstructive pulmonary disease or atrial fibrillation.

Please participate in these important learning opportunities offered under a grant from the Mother Cabrini Health Foundation in partnership among HCA Education and Research, the Healthcare Association of New York State and Iroquois Healthcare Association.
