The session featured a partnership between Nathan Littauer Hospital and Community Health Care Center to screen and assess a patient’s condition upon entry to the home with metrics for immediate triage, diagnosis and treatment.

Next Session: October 15

Don’t miss our next session, this week (on October 15), featuring University of Rochester Medical Center (URMC) and URMC Home Care.

They’ll be discussing a collaboration between the hospital ICU physician leadership and home care working with patients and families from the hospital stay through recovery at home to address Post Intensive Care Syndrome (PICS) — a potential array of physiological and mental/cognitive symptoms caused by COVID-19 that may occur during or after treatment.

These webinars are free of charge, supported by a grant from the Mother Cabrini Health Foundation. They feature a pairing of hospital and home care faculty who’ve implemented an innovative program model of collaboration to share with peers for adaptation on a statewide basis.

HCA, the Healthcare Association of New York State (HANYS), and Iroquois Healthcare Association (IHA) are project partners in this initiative.
