The next session in our Hospital-Home Care Collaboration Series is coming up this Thursday, November 19, featuring Gurwin Health Care System, Stony Brook Hospital and Stony Brook Physician Practice discussing their Integrated Care and Care Management Collaborative.
Using virtual connections, telehealth monitoring, and a clinical dashboard, this initiative joins home care and physicians in monitoring pulse-ox, cardiac tests and other readings or reports for care management of patients diagnosed with COVID-19, congestive heart failure, chronic obstructive pulmonary disease or atrial fibrillation.
