Funds Still Available to Offer Virtual Senior Center Option for Your Patients

Situation Report | April 12, 2021

Start-up funds are still available, but for a limited time, to enroll your home care patients into the Virtual Senior Center (VSC).

The VSC combats social isolation. It enables homebound older adults to connect with peers, volunteers, professional caregivers, and community members in virtual programming.

The Mother Cabrini Health Foundation has provided a grant to HCA’s 501(c)(3) in partnership with the VSC vendor, Selfhelp Community Services, to expand access to the VSC during the pandemic. The grant includes start-up funds that defray the cost for enrolling home care patients across New York State. This is a great option for providers who may wish to try out the VSC for even a few patients as you consider potential wider application with your caseloads.

However, the grant expires on June, 30, 2021, and home care providers should act as soon as possible to enroll interested patients well before the grant expiration period. Patients will have an opportunity to continue the VSC for as long as they’d like after the grant’s expiration.

To learn more about this grant-funded opportunity, please visit https://hca-nys.org/vsc and see the section titled “Start-Up Funds Now Available.” Or contact Selfhelp directly to discuss enrollment of your patients by completing the Community Partner Form here.
