Join Us on Oct. 16: Onboarding Your Patients or Clients to the Statewide Virtual Senior Center (VSC)

Situation Report | October 13, 2020

Register today for an October 16 Zoom presentation to learn how your home care, hospice or MLTC organization can onboard patients/clients so they can connect to the Statewide Virtual Senior Center (VSC).

The session, from 11 a.m. to noon, will be hosted by HCA Education and Research (HCA E&R) and Selfhelp Community Services, building on an introductory webinar that we held in July.

To register for the October 16 Zoom session, just complete the online form here and HCA E&R will e-mail you a Zoom link on the morning of the program so that you can connect to the discussion.

Selfhelp VSC experts will detail the specific steps for agencies and health plans to enroll patients or clients into the VSC, a mechanism that virtually connects and supports isolated individuals at home in the community.

Selfhelp will also provide a video demonstration of patient/client onboarding and unveil the new robust VSC-2, a platform enhancement that accommodates statewide capacity and access to the VSC as well as diversified content and program/client support.

VSC-2 and Statewide VSC access are made possible through a grant from the Mother Cabrini Health Foundation.
