Last year, 230 home care and hospital organizations and roughly a thousand participants engaged in learning sessions on trailblazing initiatives as part of our grant-funded Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond.
HCA’s program partners on this initiative are the Healthcare Association of New York State (HANYS) and Iroquois Healthcare Alliance (IHA).
These HCA-HANYS-IHA sessions and supporting resource webpage continue to offer concrete models from around the state, showcasing programs working to improve hospital-home care synchronization.
We are now building on last year’s success with a next-stage program, thanks again to a grant awarded to HCA by the Mother Cabrini Health Foundation.
HCA, HANYS and IHA have begun planning work throughout the past several weeks, curating new prototype collaborative models — including technical assistance “how-to” components — that will be featured in a series of upcoming webinars beginning in September and running through 2022. Additionally, the initiative will include a hospital-home care focus on workforce support, in-home COVID-19 testing, and other areas supporting pre-acute/prevention and post-acute/optimization of home care.
HCA, HANYS and IHA will also be engaging with state officials to help promote support for the effort along with policies that incentivize collaboration. A statewide summit is also planned.
The initiative highlights the value and power of home care and hospitals working together as core partners, and its impact on health care quality, cost-effectiveness, and health system transformation.
