HCA-HANYS-IHA Hospital-Home Care Collaborative Presents 8 New Upcoming Webinars, Summit & More

HCA, the Healthcare Association of New York State (HANYS), and Iroquois Healthcare Alliance (IHA) have joined forces for Phase II of our Statewide Hospital-Home Care Collaborative.

Please join us and be a part of this important cross-sector conversation and action that will elevate and enhance the work you do for patients.

This next stage of our program includes a series of upcoming webinars featuring successful prototype models of collaboration developed by hospital and home care partners in every region of the state.

The upcoming Collaborative webinars are accessible at no cost to any hospital and home care administrators in New York State thanks to a grant from the Mother Cabrini Health Foundation supporting this initiative.

First Session on September 9

The first program, on September 9 at noon, features an acute-care-at-home model developed in the COVID-19 surge by Catholic Health (Long Island) and Catholic Home Care. A total of eight webinars are currently scheduled, through March 10, 2022.

Statewide Summit

The series will also include a Statewide Summit on December 2, 2021 with perspectives from state officials, a panel on lessons-learned, and more. This is an

incredible opportunity to connect with system partners and join a conversation to influence state policymaking in supportive ways.

More than 230 hospital and home care organizations and 1,000 participants engaged in learning sessions during Phase I of the Collaborative last year. This second phase of the Collaborative builds on that success. Please join us.
