Hospice Item Set V.3.0 Now Available

The final Hospice Item Set (HIS) Manual version 3.00, updated HIS Admission, and HIS Discharge documents are now available here

The updates to the HIS Manual and HIS Discharge instrument reflect the removal of Section O and associated measures. The HIS Admission instrument was updated to reflect the current reporting year dates.

These changes were made to accommodate the transition to the claims-based Hospice Visits in Last Days of Life (HVLDL) measure which indicates the proportion of patients who have received visits from an RN or medical social worker (non-telephonically) on at least two out of the final three days of the patient’s life. 

The HIS Manual version 3.00 is effective as of February 16, 2021. The U.S. Centers for Medicare and Medicaid Services (CMS) notes that hospices may use either version 2.00 or 3.00 of the HIS-Discharge instrument. Both are accepted by the ASAP system. 

CMS has also added to the Quality Measure Development webpage. It provides information on the measure lifecycle and the pre-rulemaking process.

Hospices are expecting to see more information about the HVLDL measure, public reporting, and Hospice Quality Reporting updates in the upcoming Fiscal Year (FY) 2022 hospice proposed rule, expected in April.
