ICYMI: Northwell Home Care Program Featured at Inaugural HCA-HANYS-IHA Collaborative Session, with 6 More Webinars to Come

For the inaugural webinar in our hospital and home care collaboration initiative, HCA, the Healthcare Association of New York State (HANYS) and Iroquois Healthcare Association (IHA) hosted Northwell Health on September 17 to discuss its home care initiative for people infected with COVID-19.

These programs uniquely feature a pairing of home care and hospital providers to discuss innovative models working together across settings to address COVID-19 care needs and beyond. The program is supported by a grant from the Mother Cabrini Health Foundation.

For the September 17 program, Northwell outlined its COVID Ambulatory Resource Support (CARES) program and the Coronavirus Related Outpatient Work Navigators (CROWN) program which are designed for patients with symptoms that range from moderate to severe, and offer a way to treat people in the lowest-risk environment possible while also protecting hospital capacity if a second surge of COVID-19 cases develops.

Our next session, on October 8, will feature Nathan Littauer Hospital and Community Health Care Center of St. Mary’s and Nathan Littauer Hospital to discuss their High Risk/High Need Patient Collaborative, a partnership between the home care teams and primary care physicians to screen and assess a patient’s condition upon entry to the home with metrics for immediate triage, diagnosis and treatment.
