The next webinar in our Statewide Hospital-Home Care Collaborative is this Thursday, December 3, featuring Mount Sinai South Nassau and South Nassau Home Care’s Pre-acute/Post-acute Collaborative.

Click here at 4 p.m. on December 3 to join us. During this program, you’ll learn how the home care and hospital system are working together to target individuals diagnosed or frequently readmitted to the hospital for the following conditions: end-stage chronic obstructive pulmonary disorder; diabetes; pre-and post-operative care in orthopedics (e.g., hip and knee replacements) or open heart surgeries, with a focus on patient and family teaching, short stay or same-day transition back home, and recovery or rehab care at home.
