The state Department of Health (DOH) has added a new required item, Mode of Assessment, on the UAS-NY.
It has been added to Section A: Intake/History node of the Community Health Assessment (CHA), the Pediatric Assessment 0-3 (Ped 0-3) and the Pediatric Assessment 4-17 (Ped 4-17). This item captures the manner in which the assessment was conducted by the Nurse Assessor: in-person or Interactive Video Teleconference.
This action was taken as part of DOH’s policy to continue to allow for community health assessments to be completed via a telehealth modality.
