Situation Report | October 5, 2020

Please join us on Thursday, October 8, for the next webinar in our “Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond.”

Thursday’s session (from 4 to 5 p.m.) features Nathan Littauer Hospital and Community Health Care Center of St. Mary’s and Nathan Littauer Hospital. This partnership between the home care teams and primary care physicians is working to screen and assess a patient’s condition upon entry to the home with metrics for immediate triage, diagnosis and treatment.

To join the webinar at 4 p.m., click here. You can also add this session — and all others in the webinar series — to your personal calendar as a reminder to join. Just go to our Collaborative webpage to access calendar links.

All sessions in this series are free of charge, supported by a grant from the Mother Cabrini Health Foundation. They feature a pairing of hospital and home care faculty who’ve implemented an innovative program model of collaboration to share with peers for adaptation on a statewide basis.

HCA, the Healthcare Association of New York State (HANYS), and Iroquois Healthcare Association (IHA) are project partners in this initiative.
