Over 300 Providers Participate in HCA, HANYS, IHA, DOH Hospital-Home Care Collaborative Launch

HCA and partners launched the HCA-originated “Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond” during a September 3 webinar to over 300 hospital and home care representatives. You can view the webinar archive here.

Leaders of HCA, the Healthcare Association of New York State (HANYS) and the Iroquois Healthcare Association (IHA) outlined the Collaborative’s background, components and target outcomes for the coming weeks and months.

Paul Francis, newly appointed to his role as Senior Advisor to the Commissioner of Health, opened the program, along with the Governor’s newly appointed Assistant Secretary for Health and Human Services Jennifer Rentas.

The statewide initiative is supported by a grant from the Mother Cabrini Health Foundation to advance statewide systemic collaboration in pre-acute and post-acute care for COVID-19 and beyond to improve quality, access, outcomes, patient experience and cost-effectiveness.

The initiative includes several components. Seven upcoming webinars will showcase prototype hospital-home care collaboratives. The webinars will be accompanied by resources and tools, technical assistance, engagement of state officials for assistance with regulatory and procedural needs, reporting and other elements.

Following the September 3 launch webinar, HCA and partners circulated a registration link for the next sessions in the series, starting on September 17 with Northwell Health System presenting on its efforts to prevent hospitalization of COVID-19 cases and related high-risk patients through collaboration with home care.
