As we notified HCA members in an alert last week, high-level local and regional authorities have requested information on the vaccine status and needs of home care workers in an effort to facilitate vaccine access.
To collect this information, HCA, the New York State Association of Health Care Providers (HCP), and LeadingAge New York urge your organization to complete a very brief survey here on the approximate number of home care workers you employ who are not yet vaccinated.
This brief, eight-question survey will take approximately five minutes to complete and responses are due by close of business on Wednesday, February 17.
All responses are confidential and will only be reported in the aggregate. HCA, HCP and LeadingAge New York are all asking our respective home care members to complete the survey. If your organization receives this notice from more than one association, please submit only one survey for your organization.
Please also note that completion of this survey is for information purposes to help direct decision-making on vaccine access and, therefore, does not guarantee access to vaccine or supply.
Your response to this survey, however, will help assist efforts in directing vaccine to the appropriate sites for phase 1A home care workers. Please answer this survey as soon as possible.
