Save the Date! New “Hospital-Home Care Collaborative” Webinar on September 29

Please join HCA, HANYS and IHA for a presentation of our Statewide Hospital-Home Care Collaborative, on September 29 at noon, featuring Mohawk Valley Health System (MVHS) and Senior Network Health MLTC, part of MVHS’s Home Care Division.

MVHS and Senior Network Health MLTC are using a Mobile Point-of-Dispensing (POD) team to reach underserved populations with COVID-19 vaccines.

The Mobile PODs have already provided 1,325 vaccines while strengthening critical partnerships with local communities and community organizations.

In this webinar, participants will learn how to create a mobile team using organizational assets and how to integrate this model into community health improvement initiatives, particularly those that are addressing health disparities.

About the Collaborative 

With the major thrust of the federal, state and industry policy and trends all pivoting on system collaboration and integration, be right on the cutting-edge of state-of-art practice by being part of the Statewide Hospital-Home Care Collaborative! 

HCA and our state and regional hospital association partners, the Healthcare Association of New York State (HANYS),and Iroquois Healthcare Alliance (IHA), have joined forces to create this opportunity for home care, hospitals and key partners under Phase II of our Statewide Hospital-Home Care Collaborative. 

As part of this initiative, we are presenting to core provider partners statewide a series of web sessions that feature prototype models of collaboration, with faculty from the partnering providers describing the development, design and experiences of their collaborative. Each of these prototypes is presented with the goal of making the information available for emulation of the model. 

There is no charge for these sessions, which are provided with the generous support of a grant to HCA Education and Research by the Mother Cabrini Health Foundation.
