Stony Brook University Hospital Seeks Patient Discharge Planning Services

Stony Brook University Hospital is seeking an organization to provide patient discharge planning services to assist patients with a transition to skilled nursing facilities, rehabilitation facilities, hospice, or a patient’s home with home care services. In addition, an organization must be able to provide patient services for post-acute services, such as Durable Medical Equipment (DME), IV Infusion, and transportation services. The organization must have the ability to provide data reports to measure required compliance and benchmark goals.
