HCA Education and Research (HCA E&R) and partners held the third in our series of Home Care-Hospital Collaboration webinars last week featuring the University of Rochester Medical Center (URMC) and URMC Home Care.

All webinars in this series include a live component as well as an archive available on-demand immediately after the live session.

The UR Medicine program discussed how the hospital ICU physician leadership and home care agency are working with patients and families from the hospital stay through recovery at home to address Post Intensive Care Syndrome (PICS).

Next Session on Nov. 5: eMOLST Physician-Hospital-Home Care Collaborative

Our next session is on November 5, featuring Dr. Patricia Bomba of NYU Hospital and Visiting Nurse Service of New York to discuss a joint effort to leverage Medical Orders for Life-Sustaining Treatment (MOLST) and eMOLST during the COVID-19 pandemic to ensure patients’ preferences are honored, especially during a surge.
