2023 Public Health Partnership Conference

HCA joined nearly 400 Public Health leaders at the 2023 Public Health Partnership Conference (PHPC) in White Plains, New York hosted by The New York State Public Health Association, New York State Association of County Health Officials and New York State Association for Rural Health.

This year’s conference stressed the importance of building healthy communities. HCA heard from industry experts on various public health issues, solutions and best practices. Keynote speaker Dr. Brian Castrucci reminded us of the importance of rebuilding healthy communities and what that looks like post pandemic. Now more than ever we need to rebuild the trust of our communities to support, promote and advocate the health and well-being of all individuals across the state of New York and beyond.

Arianna Stone, Director of Research and Development and Kaylee White, Associate for Public Health and Policy, presented HCA’s grant initiative Addressing Health Disparities Through Home Care at this year’s PHPC conference.

HCA conducted a brief survey to explore interventions that help identify and address health disparities impacting care in HCBS. The goal of this survey was to identify the types of health disparities experienced by the populations you serve or interface with and identify potential interventions and best practices that can help eliminate persistent gaps in care.

In an effort to eliminate these persistent gaps in care HCA has developed the following initiatives that are available statewide for all home care and hospice providers:

Diversity, equity and inclusion: A custom-curated portfolio of on-demand educational training was created for home care and hospice providers. This training focused on addressing societal issues that will engage employees by helping them understand, recognize, and advocate for a diverse, equitable, and inclusive culture.

Mental health: A special training and curriculum was developed to advance the skills and knowledge of agencies’ clinical staff enhancing care of individuals with co-occurring physical and mental/behavioral health needs.

Translation services: In conjunction with translation services, cards were developed in different languages containing a translation line for use by patients in healthcare settings.

Population health care coordination: Population health care coordination: This program trains and certifies practitioners in enhanced patient-centric, organizational, and population-centric analytical direct-care management skills.
