The Health Care Personnel Influenza Vaccination Report will open on Friday, April 1, 2022 and must be electronically submitted to the state Department of Health (DOH) by June 1, 2022.
All facilities covered under New York Codes, Rules and Regulations, Title 10, Section 2.59 (10 NYCRR Section 2.59), including home care and hospice providers, are required to document the number and percentage of personnel vaccinated against influenza for the current season.
This information is contained in a DOH Dear Administrator Letter (DAL) that also includes an instruction card for guidance on how to access, complete and submit the report on the Health Electronic Response Data System (HERDS) and Frequently Asked Questions (FAQs).

This year’s report covers health care personnel employed by or affiliated with your facility or agency from October 1, 2021 through March 31, 2022. Your facility or agency should enter available data in April and then resubmit as often as is necessary as new personnel join your facility or agency and/or submit documentation of vaccination.
The report may be accessed on the HERDS vis the Health Commerce System.
A pre-recorded webinar training session for the report is available on the flu section of the DOH website under the heading “Training.” DOH advises that the person completing the report view the pre-recorded webinar and reviews the FAQs.
