The state Department of Health has issued revised guidance clarifying the eligibility and appropriateness criteria for enrollment in a Health Home. They include Eligibility Requirements: Identifying Potential Members for Health Home Services and Eligibility Requirements: Identifying Potential Members for Health Home Services Appropriateness Criteria.
