Extended MLTC Acquired by Hamaspik Choice

Effective August 1, 2023, Extended MLTC’s Partial Capitation membership will be acquired by Hamaspik Choice, another Managed Long Term Care (MLTC) plan.  

Starting August 1, 2023, Extended MLTC will no longer participate as a Partial Capitation MLTC plan. According to state Department of Health data, Extended MLTC had 5,745 enrollees (April 2023) in New York City and Long Island. 

Notices to Extended MLTC’s Partial Capitation members will be mailed beginning May 23, 2023, 60 days prior to the Hamaspik Choice August 1, 2023 transfer effective date. The notices to Extended members will explain the transfer, the member’s right to select another MLTC plan and how to contact New York Medicaid Choice for assistance.   

In accordance with state Department of Health MLTC Policy 17.02, note that the receiving plan must continue to provide services under the Extended enrollee´s existing Person Centered Service Plan (PCSP), and utilize existing providers, for the earlier of the following: (i) one hundred twenty (120) days after enrollment; or (ii) until the new plan has conducted an assessment and the Extended enrollee has agreed to the new PCSP.   

Nursing home residents shall be allowed to remain in their nursing homes and be accommodated through an out–of–network arrangement if the nursing home is not part of the receiving plan´s network.
