December 2: First-of-its-Kind Summit on Transformative Collaboration

Register NOW for the December 2 Grant-Supported Statewide Summit on “Transformative Models of Collaboration,” co-hosted by HCA, HANYS and the Iroquois Healthcare Association 

Collaborative models are proving to have transformative potential in health care, and indeed are paving the way for new state-of-the-art methods service delivery and financing. 

Explore the possibilities, potential partnerships for your organizations and the future vision with NYS health officials, health industry leaders and models programs, in our upcoming, first-of-its-kind Summit, Transformative Models of Collaboration.  

This virtual summit is supported by a grant to HCA by the Mother Cabrini Health Foundation, and being co-conducted by HCA, our 501(c)(3) affiliate arm, HCA Education and Research, the Healthcare Association of New York State (HANYS)/Healthcare Educational and Research Fund and the Iroquois Healthcare Association (IHA). Providers, officials and organizations across the state will be participating and all are strongly encouraged to attend.

In 2020, HCA spearheaded a partnership with HANYS and IHA to establish the Statewide Hospital-Homecare Collaborative, supported by funding from the Mother Cabrini Health Foundation. The funding for the program was awarded to HCA Education and Research. The program, featured collaboratives, upcoming models and collaborative resources can be accessed at the HCA Innovations website.

2020’s successful launch was rewarded with a 2021 phase II grant to HCA E&R and partners, and since was grant-approved into 2022, to continue to further build on the initiative to support hospitals, home care agencies, physicians, hospice and other continuum partners to institute collaborative models of service. The goal is a coordinated and strengthened primary, pre- and post-acute health system optimizing collaboration as the core vehicle.   

The December 2 Summit is part of HCA, HANYS and IHA’s Phase II initiatives under the Statewide Hospital-Homecare Collaborative.
