New Training Resources on Chronic Conditions

The U.S. Department of Health and Human Services (HHS) has announced new training material to help provide care for people living with multiple chronic conditions.

The resources support goal three of the HHS Strategic Framework on Multiple Chronic Conditions (http://www.hhs.gov/ash/initiatives/mcc/mcc_framework.pdf), released in 2010, which is to provide better tools and information to health care, public health and social services workers who deliver care to people living with multiple chronic conditions.

The resources available online (http://www.hhs.gov/ash/initiatives/mcc/education-and-training/index.html) include:

  • “Multiple Chronic Conditions Education and Training Repository” – a searchable database of existing educational resources that specifically address the care of persons living with multiple chronic conditions;
  • “Multiple Chronic Conditions: A Framework for Education and Training” – a conceptual model that outlines the core domains and competencies for the interprofessional health care team and;
  • “Education and Training Curriculum on Multiple Chronic Conditions” – a web-based course consisting of six modules.
