New Patient and Provider Materials Available in Multiple Languages

The Home Care Association of New York State Education and Research (HCA E&R) received a multi-year, multi-component grant from the Mother Cabrini Health Foundation, Addressing Health Disparities Through Home Care.

Through this grant, HCA E&R piloted an over-the-phone point-of-care translation services in five agencies. To enhance the point-of-care service, HCA E&R with assistance from member agencies and pilot participants identified a few key documents that would be helpful to have in multiple languages that re prevalent in New York state. HCA E&R worked with LanguageLine Solutions to have these documents translated supported by funding from the Mother Cabrini Health Foundation. Special thanks goes to Nascentia Health and its incredible staff, specifically: Joann Racino, director of community health & clinical education program; and HCA Board Member Andrea Lazarek-Laquay, chief clinical officer.

Patient and provider materials available include:

  • Call for help tool
  • COPD action plan
  • General health booklet
  • Heart failure symptom tracker
  • Respiratory zone tool

Sepsis Tool and Zone Tools

In addition, HCA E&R, and core partners, (Sepsis Alliance, END SEPSIS The Legacy of Rory Staunton, IPRO, and medical students) worked to develop tools for sepsis screening and intervention in the home and community settings. All HCA members should engage in sepsis awareness, intervention, and prevention. If you are not currently using the tools, contact tperre@hcanys.org. Through the Addressing Health Disparities Through Home Care grant, HCA E&R worked to get the sepsis educational zone tools translated to multiple languages. The educational zone tools include the adult zone tool, pediatric zone tool, and maternal zone tool.
