Reminder: Limited Supply of Vaccines Available to ‘Patient-Facing’ Care Staff in NYC through HSS

HCA reminds providers that Hospital for Special Surgery (HSS) in Manhattan is making available a limited supply of COVID-19 vaccines to front-line “patient-facing” home care workers employed by home care agencies that serve patients in New York State.

The vaccine will be administered in Manhattan (see below address) and you or your staff can register for the vaccine here, or you can e-mail the following information (using the downloadable Excel spreadsheet here) for all staff requesting vaccinations through this process (please send this spreadsheet to Jennifer O’Neill at the Hospital for Special Surgery):

  • Employee name
  • Employee date of birth
  • Employee e-mail
  • Employee cell phone number
  • Occupation
  • Employer

Any scheduled appointments are dependent on HSS receiving an adequate supply of vaccines on a weekly basis.

Once the appointment has been scheduled, the home care worker will receive a call from HSS to be screened and formally registered in the system for the COVID-19 vaccine.

Home care workers will need to bring documentation of their employment (ID badge or recent pay stub); a driver’s license, passport, or other government-issued identification; and a health insurance/Medicaid card (if any) to HSS on the day of their vaccination.

At the scheduled day and time of the appointment, the home care worker will go to 525 East 71st Street (between York Ave. and FDR Drive), New York, New York 10021.

Upon entry into the Belaire Courtyard at 525 East 71st Street, the home care worker will present the above required documentation.
