Revised NHTD Waiver Materials Now Available

The state Department of Health (DOH) has revised its website on the Nursing Home Transition and Diversion (NHTD) waiver program

Located on the site is information on how to become a waiver service provider or participant, training materials for providers, forms related to NHTD waiver services and Reference materials that include the Program Manual and the 1915(c) waiver application. Many of the forms are now fillable and translations are available. The forms are located on a separate tab and are not included in the Program Manual content.

The protocols established in the Program Manual are now in effect. Providers are advised to review the Manual closely as there are changes in timelines, enhanced service definitions and new information.  

Note that the revised program manual now provides that all service notifications (previously referred to as a “10-day letter”) are now on a 30 day scheduled comparable to Service Coordination. 

Within in the near future DOH will schedule a “walk-through” for everyone so that it may address your questions and help facilitate implementation of any changes. 

DOH is working on the development of a comparable site for Traumatic Brain Injury (TBI) and the TBI Program Manual is under revision.
