Statewide Health Care Facility Transformation Program IV Announced

The state Department of Health (DOH) has announced that up to $50 million is available under the Statewide Health Care Facility Transformation Program IV (Microsoft Word – RFA 20340 Statewide IV-NH -Final (hca-nys.org). 

The purpose of the program is to provide grants in support of capital projects directly related to residential and community-based alternatives to the traditional model of nursing home care. 

To receive funding, the Eligible Applicant must demonstrate how the proposed use of the grant will strengthen and protect continued access to health care services in communities. Eligible Applicants should describe:  

  1. Steps to be taken to transform, redesign, and strengthen the quality and innovation of residential healthcare services and programs; improve health outcomes and quality of the patient experience; create and contribute to a financially sustainable system of care; or preserve or expand the ongoing availability of essential health care services to the people of the community or communities served by the Eligible Applicant.  
  2. How the Eligible Project will maintain or improve the financial condition of the Eligible Applicant.  
  3. An estimate of the total costs of carrying out health care transformation activities (other than the Eligible Project as hereafter defined) and ability to secure potential funding for such costs.  
  4. Steps to be taken to engage the community in the development of the Eligible Project. 

Eligible applicants include home care providers, nursing homes, adult care facilities, Assisted Living Programs and others. 

Questions are due February 14, 2024 and applications are due April 9, 2024.
