When it comes to patient care, you will always find Tracy Crandall going that extra mile.
Balancing a high caseload of patients, busy family life and teaching others is all in a day’s work for her.
The “home care hero,” per her nominator, is this year’s recipient of the Home Care Champion Award. It recognizes an individual who has demonstrated a strong and model commitment to home care patients and agency partners.
Tracy is an RN Case Manager with the Eddy Visiting Nurse and Rehab Association in Troy.
She was cited for caring for her patients, as well as being the first one to volunteer for precepting new hires and allowing them to shadow her for the day.
She routinely accepts extra visits and often donates food for patients who might not have enough to eat.
Even in her off time, she volunteers to see patients if other nurses are not available.
She works well with her colleagues and is a true team player.
More than that, Tracy’s patients love and appreciate her and she has an excellent rapport with them.
During our most recent Nor’easter, Tracy was out in the snow to tend to her patients and also encouraged her fellow nurses to show their dedication and commitment to home care through social media.
In doing so, Tracy shone a light on her colleagues and the vital work they do each day, every day, no matter what comes their way.
