Funding Announced for Statewide Health Care Facility Transformation Program IV and Other Programs

On January 9, 2024, the New York State Department of Health (DOH) announced the availability of funds under the Statewide Health Care Facility Transformation Program IV 

A total of up to $250,000,000 is available through Request for Applications (RFA #20244) to health care providers in support of capital projects, debt retirement, working capital or other non-capital projects that facilitate furthering transformational goals, including, but not limited to, transforming, redesigning, and strengthening quality health care services in alignment with statewide and regional health care needs and in the ongoing pandemic response, or other activities intended to build innovative, patient-centered models of care, to increase access to care, to improve the quality of care and to ensure financial sustainability of health care providers. 

Eligible Applicants include hospitals, residential care facilities, adult care facilities, assisted living programs, children’s residential treatment facilities licensed by the Office of Mental Health (OMH), and residential facilities or day program facilities licensed by the Office for People With Developmental Disabilities (OPWDD).  Eligible Applicants also include community-based health care providers which include diagnostic and treatment centers, home care providers, primary care providers, independent practice associations or organizations, hospice providers, outpatient clinics licensed by OMH, the Office of Addiction Services and Support (OASAS) or OPWDD, and community-based programs funded by OMH, OASAS, OPWDD or through local governmental units.   

The Statewide Health Care Facility Transformation Program IV RFA can be accessed through the DOH website at http://www.health.ny.gov/funding/, through the Grants Gateway website at http://grantsreform.ny.gov or directly at Microsoft Word – RFA 20244 Statewide IV RFA 1.9 final (hca-nys.org).  Applications must be submitted in Grants Gateway by March 26, 2024.

Transformation Program IV – Residential and Community-Based Alternatives to the Traditional Model of Nursing Care 

DOH has also announced that up to $50 million is available under the Statewide Health Care Facility Transformation Program IV  – Residential and Community-Based Alternatives to the Traditional Model of Nursing Care (Microsoft Word – RFA 20340 Statewide IV-NH -Final (hca-nys.org).  HCA had included information on this in our January 2, 2024 Situation Report. 

The purpose of the program is to provide grants in support of capital projects directly related to residential and community-based alternatives to the traditional model of nursing home care.  

Questions are due February 14, 2024 and applications are due April 9, 2024. 

Transformation Program IV and V Health Information Technology, Cybersecurity, and Telehealth Transformation 

DOH has also announced the availability of funds to support technological and telehealth projects that facilitate health care transformation activities. Funding will be made available in the form of grants and can include capital, non-capital, and working capital expenses. Some areas covered include: electronic health records; cybersecurity; telehealth; and health management tools.
